Everyone, particularly seniors, should have an Advance Health Care Directive. It is the legally recognized document used to ensure that your health care wishes are known and considered if you are unable to speak for yourself.  We will complete an advance health care directive for you free of charge.  This clinic is only available to Community Heritage Apartment residents.
